Self-Administered COVID-19 Screening Consent Form and Waiver

This consent provides NextEra Energy, Inc. and its subsidiaries (“NextEra”) with your permission to participate in a COVID-19 screening procedure based on NextEra’s need to maintain a safe environment for employees, contractors, vendors, and other essential persons with whom you may come into contact.  By signing below, you are indicating that you consent to this procedure for the detection of COVID-19.  

The test being administered involves a finger-prick to obtain a small blood sample which will then be tested for the presence of antibodies that may indicate the presence of COVID-19.Based on the results, you may not be allowed to enter the NextEra facility.  

This test has been allowed for use by the Food and Drug Administration (“FDA”) but has not been approved by the FDA.  This test alone may not be sufficient to detect or rule out the possibility that you have been exposed to or are infected with COVID-19. You should carefully monitor your own symptoms and, notwithstanding the results of any testing, you must stay home and consult with your physician if you experience symptoms of COVID-19.

You have the right to discuss the proposed testing with your physician, to learn about the purpose, potential risks and benefits of any testing. Based upon your test results, if you are denied entry to the NextEra facility, you should contact a physician or other medical professional for advice.  Because of the ongoing public-health crisis, it may be necessary for NextEra to share the results of your test with public health authorities.

By signing below, you consent to the disclosure of such information to NextEra and, as requested, recommended or required by federal, state, and local public health authorities.

Where do you ship?

We currently ship our COVID Antibody tests to all continental US states except New York due to state regulatory laws. We're actively working to receive approval in New York. We don't currently ship to Canada or any other countries outside of the US.

How do I receive my results as a patient?

You will receive your results through the imaware™ digital platform. When your results are ready, you'll receive an email notifying you. You can then login and view your results by logging into the patient portal at https://portal.imaware.health/. Your digital results are also readily available to be downloaded as a PDF.

How long will it take to get my kit?

When you place an order with us, we begin to package your collection kit immediately.  When orders are placed before 2pm Central Time, we ship those kits out the same day.  For orders placed after 2pm, we ship the next day.We utilize USPS First Class shipping, and your kit will be shipped to you within 5 business days. In fact most of our customers get their kits within 3 business days.

Are my test results stored securely?

Yes, your imaware™ account is secure. We have built leading-edge encryption and security features, and our digital health platform is HIPAA compliant. For more details about our privacy policy, please click here.
